For Our Patients
1. Look for an email from Patient Fusion.  Be sure to check ALL inboxes of your email account, as the initial email may go to a junk or spam folder.
2. Click the "Access Your Records" link located within the email.
3. Select "Request Security via Phone"
4. Enter your date of birth and phone number
5. You will receive a code via text or voice, dependent on the selected method.
6. Enter code
7. Select "Create A New Account"
8. You will be prompted to fill out several required fields.
9. Select and answer a security question
10. Create a username and password.  Keep in mind, your password must be a minimum of 8 characters and contain a mix of uppercase letters and numbers or symbols.
11. Click "Create A New Account"
12. You are now logged into your personal account.  You can manage your account and view health records, use secure messaging, and book appointments.
